NTUC Health Co-operative Limited (NTUC Health) is an NTUC social enterprise that provides a comprehensive and integrated suite of quality and affordable health and eldercare services to meet the growing needs of families and their dependents. Building on close to three decades of experience and expertise, NTUC Health is among the largest providers of active ageing, senior day care, nursing home, and home personal care services in Singapore. We also serve vulnerable seniors in selected areas in Singapore through Community Case Management Services (CCMS), CREST mental health support, and a sheltered senior group home. In addition, we run a family medicine clinic.

Job Role Summary:
Information and Referral (I & R)
- Assist in managing walk-in, online, and phone enquiries about NTUC Health services.
- Support the team in screening and understanding clients' needs using guided tools.
- Help coordinate service referrals by preparing referral forms and following up with relevant teams.
Service Coordination
- Assist in scheduling and coordinating Home Care services for new clients.
- Support the team in maintaining accurate documentation in both systems and physical records.
- Work with internal teams to help ensure smooth service delivery.
Case Management
- Assist in conducting basic follow-up calls with clients and caregivers to check on their service experience.
- Shadow experienced staff during home visits to observe client interactions and needs assessment.
- Help with preparing reports and updating client records based on guidance from supervisors.
Service Development
- Support program development and evaluation efforts to enhance eldercare services in an evolving sector.
Qualifications
We're seeking passionate individuals who want to make a meaningful impact in the lives of seniors. You should be:
- Pursuing a Diploma or Advanced Diploma in healthcare-related or hospitality disciplines
- Proficient in Google Workspace applications

Additionally, the ideal individual should have:
- Strong interpersonal and communication skills (both written and spoken)
- A team-player mindset with the ability to multi-task effectively
- Good judgment, problem-solving, and decision-making skills
- A passion for engaging with people and working collaboratively in teams
- Patience and dedication to supporting seniors with care and compassion
- Independence and accountability, with the ability to meet deadlines
- Proficiency in English (knowledge of Mandarin or dialects is a plus!)
- A service-oriented mindset with a passion for community work
